Your Appointment
Before your appointment
Please arrive 5 to 10 minutes before your appointment to allow time for check in and any required paperwork.
You may be asked to complete a Patient Details Form before your appointment. Where possible, we encourage forms to be completed and returned in advance.
What should I bring?
Please bring:
- Your Medicare card
- Your referral letter if it has not already been sent to us
- Any relevant test results, reports or medical imaging that have not already been provided
- A list of current medications
You may also need:
- Private health insurance details (if applicable)
- DVA card and relevant approval information
- Workers Compensation claim details, including claim number, insurer and employer information
- Motor Vehicle Injury claim details, including claim number and insurer information
